CDC's National Asthma Control Program (NACP) currently funds 29 state, territorial, and municipal partners to improve the reach, quality, effectiveness, and sustainability of asthma control services and to reduce asthma morbidity, mortality, and disparities by implementing evidence-based strategies through a 4-year cooperative agreement.

Overview

These programs strengthen existing organizational infrastructure to expand the reach of services through NACP's six EXHALE strategies:

  • Education on asthma self-management; \n
      \n
    • \n
  • eXtinguishing smoking and exposure to second-hand smoke; \n
      \n
    • \n
  • Home visits for trigger reduction and asthma self-management education (AS-ME); \n
      \n
    • \n
  • Achievement of guidelines-based medical management; \n
      \n
    • \n
  • Linkages and coordination of care; and \n
      \n
    • \n
  • Environmental policies or best practices to reduce indoor and outdoor asthma triggers. \n
