Proteins that are present in cancer cells but not normal cells or that are more abundant in cancer cells would be potential targets, especially if they are known to be involved in cell growth or survival. An example of such a differentially expressed target is the&nbsp;<a data-glossary-id=\"CDR0000044570\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000044570&amp;version=Patient&amp;language=en\">human epidermal growth factor receptor 2</a> <a data-glossary-id=\"CDR0000046092\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000046092&amp;version=Patient&amp;language=en\">protein</a>&nbsp;(HER-2). HER-2 is expressed at high levels on the surface of some cancer cells. Several targeted therapies are directed against HER-2, including&nbsp;<a data-glossary-id=\"CDR0000045439\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000045439&amp;version=Patient&amp;language=en\">trastuzumab</a>&nbsp;(Herceptin), which is approved to treat certain breast and stomach cancers that&nbsp;<a data-glossary-id=\"CDR0000045812\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000045812&amp;version=Patient&amp;language=en\">overexpress</a>&nbsp;HER-2.</p> \n       <p>Another approach to identify potential targets is to determine whether cancer cells produce mutant (altered) proteins that drive cancer&nbsp;<a data-glossary-id=\"CDR0000044078\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000044078&amp;version=Patient&amp;language=en\">progression</a>. For example, the cell growth signaling protein BRAF is present in an altered form (known as&nbsp;BRAF V600E)&nbsp;in many&nbsp;<a data-glossary-id=\"CDR0000045135\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000045135&amp;version=Patient&amp;language=en\">melanomas</a>.&nbsp;<a data-glossary-id=\"CDR0000702051\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000702051&amp;version=Patient&amp;language=en\">Vemurafenib</a>&nbsp;(Zelboraf) targets this mutant form of the BRAF protein and is approved to treat patients with&nbsp;<a data-glossary-id=\"CDR0000285970\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000285970&amp;version=Patient&amp;language=en\">inoperable</a>&nbsp;or&nbsp;<a data-glossary-id=\"CDR0000044058\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000044058&amp;version=Patient&amp;language=en\">metastatic</a>&nbsp;melanoma that contains this altered BRAF protein.</p> \n       <p>Researchers also look for abnormalities in&nbsp;<a data-glossary-id=\"CDR0000046470\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000046470&amp;version=Patient&amp;language=en\">chromosomes</a>&nbsp;that are present in cancer cells but not in normal cells. Binding of the monoclonal antibody to the target molecule results in the immune destruction of cells that express that target molecule. These criteria are set by the FDA when it approves a specific targeted therapy.</p> \n       </div> \n      </div> \n     </div> \n    </section> \n    <section itemscope itemprop=\"mainEntity\" itemtype=\"https://schema.org/Question\"> \n     <h2 itemprop=\"name\"> <p>What are the limitations of targeted cancer therapies?</p> </h2> \n     <div itemscope itemprop=\"acceptedAnswer\" itemtype=\"https://schema.org/Answer\"> \n      <div itemprop=\"text\"> \n       <p>Targeted therapies do have some limitations. One is that cancer cells can become resistant to them. For example, a recent study found that using two therapies that target different parts of the cell <a data-glossary-id=\"CDR0000561720\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000561720&amp;version=Patient&amp;language=en\">signaling pathway</a> that is <a data-glossary-id=\"CDR0000044643\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000044643&amp;version=Patient&amp;language=en\">altered</a> in <a data-glossary-id=\"CDR0000045135\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000045135&amp;version=Patient&amp;language=en\">melanoma</a> by the BRAF V600E <a data-glossary-id=\"CDR0000046063\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000046063&amp;version=Patient&amp;language=en\">mutation</a> slowed the development of resistance and <a data-glossary-id=\"CDR0000045669\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000045669&amp;version=Patient&amp;language=en\">disease progression</a> to a greater extent &nbsp;than using just one targeted therapy (<a href=\"#r1\">1</a>). &nbsp; &nbsp;</p> \n       <p>Another approach is to use a targeted therapy in combination with one or more traditional <a data-glossary-id=\"CDR0000045214\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000045214&amp;version=Patient&amp;language=en\">chemotherapy</a> drugs. For example, the targeted therapy <a data-glossary-id=\"CDR0000045439\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000045439&amp;version=Patient&amp;language=en\">trastuzumab</a> (Herceptin) has been used in combination with <a data-glossary-id=\"CDR0000045382\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000045382&amp;version=Patient&amp;language=en\">docetaxel</a>, a traditional chemotherapy drug, to treat women with metastatic <a data-glossary-id=\"CDR0000444971\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000444971&amp;version=Patient&amp;language=en\">breast cancer</a> that <a data-glossary-id=\"CDR0000045812\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000045812&amp;version=Patient&amp;language=en\">overexpresses</a> the protein <a data-glossary-id=\"CDR0000044945\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000044945&amp;version=Patient&amp;language=en\">HER2/neu</a>.</p> \n       <p>Another limitation of targeted therapy at present is that drugs for some identified targets are difficult to develop because of the target\u2019s structure and/or the way its function is regulated in the cell. One example is Ras, a signaling protein that is <a data-glossary-id=\"CDR0000044133\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000044133&amp;version=Patient&amp;language=en\">mutated</a> in as many as one-quarter of all cancers (and in the majority of certain cancer types, such as <a data-glossary-id=\"CDR0000044521\" href=\"https://www.cancer.gov/Common/PopUps/popDefinition.aspx?id=CDR0000044521&amp;version=Patient&amp;language=en\">pancreatic cancer</a>). To date, it has not been possible to develop inhibitors of Ras signaling with existing drug development technologies.
